SEND Local Offer
School Contact
Our SENCo is Miss Katie Mouatt and she can be contacted by making an appointment at the school office.
At Collingwood Primary School we are committed to offering an inclusive curriculum to ensure the best possible outcomes for all of our pupils whatever their needs and abilities.
We seek to ensure that pupils with SEND are fully included in all aspects of school life. We believe that pupils with SEND and their parents/carers should be at the heart of planning and decision making. We aim to provide opportunities for pupils with SEND and their parents/carers to play an active role in planning their provision in accordance with the SEND Code of Practice 2014.
Collingwood has a clear referral process whereby teachers raise concerns with the SENCo regarding the performance of pupils who they suspect of having special educational needs.
This could be during formal assessment and review periods but concerns can also be raised at any point throughout the year.
Following a 'cause for concern', pupils who are not making progress through the graduated approach may be further assessed by external agencies, such as the Educational Psychologist, IPASS, Northcott Outreach, Whitehouse Outreach and the Speech and Language Therapy Service.
Baseline assessments are carried out as all children enter FS1 and FS2. Reading tests are used to assess children using the Lexia programme. We have a range of diagnostic tests to help us to further identify pupils’ needs. Recommendations are put in place following assessments carried out by outside agencies.
If you have any concerns regarding your child, please speak to your child's class teacher as a first point of contact as most concerns are easily addressed this way. If you would prefer to, you can talk to the SENDCo. We encourage all parents to share their concerns quickly. If you need more support in meetings at school, you can contact the Parent Partnership KIDS, who are happy to help.

The SEND Local Offer
The local offer provides information for children and young people with special educational needs (SEND) and their parents or carers in a single place.
What is a local offer?
In Hull there’s lots of support available to children with disabilities and special educational needs, and their families. You’ll find information about that Local Offer of support here.
Whether it relates to education, health, social care or parental support, and regardless of who’s providing it, we’ll aim to point you in the right direction.
The local offer provides information on what services children, young people and their families can expect from a range of local agencies, including education, health and social care.
